Navigating through the forms
The form navigation table lists all the forms that comprise the survey. You can navigate through the forms as often as you wish, as you examine the questions and gather data to complete your answers. The location indicator shows you where you are in the sequence. Your data is saved in our database each time you select the "save" "next" or "back" button on any form. The status indicator shows you what sections you have completed and what sections remain.
Take all the time you need
You don't need to complete the entire survey in one session. Take as long as you need to gather your information. You can log back on to the system and continue your work at any time until the survey deadline of April 25, 2008
Your survey is not considered complete until you press the final, "Commit Data" button.
Obtaining the Survey Report
We'll notify you by email as soon as the Survey Report is published. The anticipated date of publication is May 30, 2008.
